EMPLOYEE KILLED WHEN CAUGHT IN MACHINE

On June 13, 1988, Employee #1 and coworkers, millwrights, were called to repair a machine in the Gaylord Papermill in Bogalusa, LA. The machine involved was a 30P selectifier screen, labeled at the main power substation as middle pressure screen number 2, located on the northeast corner, basement floor, paper machine number 2. This machine is a pressure filter that operates on the principle of foils turning at a high rate of speed that forces the influent stream of water and pulp through small openings in a cylindrical drum. Its purpose is to filter out metal and trash from the stream before going to the paper machine. This machine, middle pressure screen number 2, was down due to a piece of metal caught between the foil blades and the drum. Employee #1 was caught in the machine when a lockout or tagout procedure was not followed. Employee #1 was killed.
