Employee killed when forklift flips over

At approximately 2:30 p.m. on May 26, 1994, Employee #1 was operating a Hyster forklift. As he negotiated a right turn, he lost control and the forklift flipped over and struck him. Employee #1 sustained massive head injuries and died at approximately 11:30 p.m. on May 27, 1994.
