EMPLOYEE CRUSHED BETWEEN COUNTERWEIGHT AND MAST

Employee #1, along with another employee, was conducting pre-service checks on a dual personnel hoist mounted on each side of a center mast. Both employees found the gear drives to be noisy due to the lack of adequate lubrication. Neither employee corrected the situation, but both began to use the hoists to lift personnel. During the shift, Employee #1 began to lubricate the gears of one of the hoists at ground level. He climbed over the hoist enclosure into the center mast area. The second employee was called to move his hoist from the 7th floor to the top floor of the building, but did not notice Employee #1 in the mast area. As the other employee raised the active hoist, the counterweight came down the center mast and struck Employee #1, crushing him between the counterweight and the center mast frame.
