Investigation Summary

Investigation Nr: 14470884
Event: 04/30/1986
EMPLOYEE INHALED HYDROCHLORIC ACID FUMES

THE EMPLOYEES WERE FILLING A 330 GALLON STORAGE TANK LOCATED IN THE CHEMICAL STORAGE ROOM. THE TANK OVERFLOWED, SPILLING HYDROCHLORIC ACID ONTO THE FLOOR. EMPLOYEE #1 ATTEMPTED TO CLEAN UP THE SPILL, USING ONLY A HALF-FACE RESPIRATOR AND A PAIR OF GLOVES FOR PROTECTION. EMPLOYEE #1 WAS ADMITTED TO THE HOSPITAL FOR THE TREATMENT OF HYDROGEN CHLORIDE INHALATION. AS A RESULT OF THIS ACCIDENT, THE COMPANY HAS INSTALLED THREE INTERLOCKING AUTOMATED SYSTEMS DESIGNED TO PREVENT CHEMICAL OVERFLOWS. THE TANK FILLING OPERATION WAS PERFORMED COMPLETELY MANUALLY BEFORE THE ACCIDENT.
