Investigation Summary
At approximately 4:48 p.m. on October 4, 2014, an electrician employee was engaged in the removal of some overhead lights and installation of a switch. The employee was hired, as an independent contractor at Tires & Wheels, which primarily engaged in retail tire sales and services. The employee was nearly done with the job. He had moved the overhead light, installed conduit and a switch, and when making a final connection at an electrical box on the ceiling at the north end of the shop, he fell. The employee was standing on tires that were on a tire rack. The tire rack was 7 feet high. There were 5 tires stacked one on top of another directly under the electrical box. Each tire was about a foot wide. The ceiling was 16 feet 5 inches high. The employee fell and landed head first on the concrete floor of the shop. The employee suffered head injuries due to a fall from a tire rack, onto a concrete floor and was transported by ambulance to a community hospital and then transferred to another medical center. The employee died on October 5, 2014.
