Investigation Summary
At 11:15 a.m. on October 1, 2014, an employee was disconnecting a gang saw from the electric power to relocate it to another facility. The employee contacted an energized wire and was electrocuted. The current entered the employee through his hands. The saw was powered by a 480 volt 3 phase connection. However, only one phase was energized leaving a voltage of 281 volts to ground and a 110 amp fuse.
