Investigation Summary
At approximately 12:30 p.m. on September 19, 2014, an employee was standing on a Louisville Fiberglass Step Ladder preparing to climb down to the floor. The employee was found face down after being on a ladder. The employee apparently fell striking the ground and the I beams supporting the mechanical units in the penthouse mechanical room. The medical examiners office deemed the accident a heart related issue and no apparent issues from a possible fall. The employee apparently died of heart related issues and not work related.
