Investigation Summary
At 10:50 a.m. on September16, 2014, an truck driver was directing the loading of his flatbed truck with materials. As he stood on the passenger side of his vehicle, a forklift was placing a second load on the truck from the passenger side of the vehicle. The second load inadvertently struck items from the first load, causing a metal structure to fall off the truck and strike the truck driver. The driver suffered a cut to his abdomen and a severe head injury. The truck driver was killed.
