Investigation Summary
At 12:15 p.m. on September 1, 2014, an employee was working from a stepladder to repair an overhead door opener that was mounted on the ceiling of a maintenance shop. As he was working, the employee fell 12 feet to the concrete floor, striking his head. He suffered multiple injuries to his head, including a fractured skull, lacerations and bruising of brain tissue, as well as subgaleal, subdural, and subarachnoid hemorrhages that resulted in his death.
