Investigation Summary
At about 4:00 p.m. on August 15, 2014, Employee #1 was using two jack stands to support a Chrysler PT Cruiser while he was replacing an oil feeder line to the turbo. Employee #1was laying beneath the vehicle when one of the jack stands fell over, causing the vehicle to fall onto Employee #1. Employee #1 was killed.
