Investigation Summary
At 3:53 p.m. on August 4, 2014, Employee #1 was working underneath a 2007 Nissan Altima, doing repairs at an auto repair shop. The vehicle was parked on a gravel parking lot between two other vehicles. Employee #1 had raised the vehicle with a hydraulic floor jack and placed two jack stands beneath the vehicle, one Torin Black Jack 3-Ton Jack Stand under the driver side and another 3-Ton jack stand with an illegible label on the passenger side. While underneath the vehicle, the jack stand on the driver side tipped over, causing the vehicle to drop down and pin Employee #1. Employee #1 sustained blunt force trauma to the head and chest and died.
