Employee is electricuted by electrical fan

On September 8, 2011, Employee #1, a sanitation crew member at a meat packing plant, had completed cleaning an area of the facility. The employee had hosed down the area and had brought in a large fan that is used to blow water from the walls and ceilings. Without actuating the electrical disconnect or wearing any electrical personal protective equipment such as gloves, Employee #1 took the male fan plug, with a NEMA configured 240 volt plug, and was attempting to plug it into a NEMA configured 480 volt cabled receptacle. The employee was very wet from the cleaning activities and as the employee attempted to force the two mismatched connectors together brought them to his abdomen where he managed to get them engaged enough to receive a 480v shock resulting in an electrical entry burn at the navel and an exit burn on his right gluteus maximus. Employee #2, a sanitation crew member, found employee #1 and actuated the electrical disconnect and called for help. Medical responders pronounced the employee dead at the scene.
