Investigation Summary
At 5:40 p.m. on June 23, 2014, an employee and coworker loaded a salvage vehicle onto a trailer using a RT-706H Ingersoll Rand forklift and separated once the vehicle had been loaded. The coworker was driving the forklift from the rear of the salvage yard, felt a bump with the right rear tire, and saw that the employee was hanging from the running boards. The coworker reversed the forklift, stabilized it on all wheels, and the employee got up and walked towards another vehicle. Emergency Services were contacted and the employee was killed from internal injuries.
