Investigation Summary
At 10:30 a.m. on June 19, 2014, Employee #1 was cutting down a tree with a chain saw while using an aerial lift. The employee was not wearing fall protection and fell 30 feet from the basket of the aerial lift to the ground. Employee #1 was unconscious and unresponsive, sustaining internal injuries and was killed.
