Employee caught between a door is killed
At 6:30 a.m. on May 29, 2012, Employee #1 was standing at the entrance to a roll-off vacuum container tank while Employee #2 removed residual waste toothpaste. The door to the container suddenly closed, crushing and killing Employee #1.
