Investigation Summary
On May 28, 2014, an employee was attempting to install a power factor correction that is designed to balance the electrical system. The employee was using basic tools such as a voltage tester, screwdriver, pliers and a wire stripper/cutter. While the employee was installing electrical equipment in a 480-volt three phase system with at least 800 amps or more. There was a couple of electrical explosions followed with an arc flash. The employee was not wearing fire retardant clothing. The employee experienced second and third degree burns on 60 percent of his lower extremities later and was killed from his injuries.
