Investigation Summary
At 12:30 a.m. on May 22, 2014 an employee was repairing a malfunctioning sensor for the aggregate bins that had malfunctioned earlier that evening. The employee was inside the guardrails at the top of the plant, repairing a sensor for the aggregate bins and the "turn head" (diverting equipment) that takes the aggregate from the conveyor and distributes it to the bins. The turn head crushed the employee against a metal bar that was attached to the guard rail. The employee was later found dead by a co-worker. He was caught between the turn head and the guard rail.
