Investigation Summary
At 2:30 a.m. on May 11, 2014, the employees were attaching the counter weights while assembling a CK Kobelco 1600 Crane at the time of the accident. A vehicle by-passed barricades and local police which were positioned miles ahead of the work zone. Employee #1 was directly hit by the vehicle throwing him into an outrigger. Employee #2 was caught-in/between the vehicle and counter weight. Employee #1 was killed and employee #2 suffered an amputation.
