THE FOREMAN WAS CHECKING ON HIS EMPLOYEES, WHO WERE GRADNG AND MOVING EARTH. A BUCKET LOADER WAS OPERATING NEARBY, LOADING A DUMP TRUCK. THE LOADER OPERATOR BACKED UP, STRIKING AND RUNNING OVER THE FOREMAN. THE BACKUP ALARM WAS NOT OPERATING AT THE TIME.
