Investigation Summary
At approximately 11:00 a.m. on July 19, 2011, an employee was setting transfer baskets in preparation of concrete pavement to be poured. The employee was wearing a high visibility vest and a hard hat. The employee received limited training on heat stress. The weather on the day and time surrounding the incident was zero precipitation, high humidity, temps were at approximately mid to upper 80 degrees (F) at 9:00 AM, 90 degrees (F) by 10 AM and temps rose to a high of 97 degrees by approximately 2 PM. The employee began showing symptoms at around 10:30 AM, which included but were not limited to dizziness, loss of balance, and was otherwise non responsive. Emergency response call was received at 12:51 PM and responders arrived on site at 12:53 PM. The work being performed was a normal routine portion of the employee's job. The employee suffered effects as a result of heat stress related disorders, and died based from exposure to high temperatures resulting from heat stroke.
