Investigation Summary
At approximately 1:35 p.m. on March 22, 2014, Employee #1 was felling a tree preparing the site for the right of way for a pipeline using a saw. The tree being cut down had an adjacent tree leaning into it and intertwined in its top branches by vines. When the tree was felled, the adjacent tree fell onto Employee #1 in his egress path, pinning him to the ground. Employee #1 was killed with crushing injuries to his right side.
