Investigation Summary
At 11:30 a.m. on January 16, 2014, an employee was taking bolts out of a bucket next to the Acrow Bridge Panel while a coworker was standing on the bridge panel, removing the braces. When the coworker removed the last brace the panel fell, crushing and killing the employee.
