Investigation Summary
At 11:00 a.m. on January 11, 2014, an employee was killed when he was caught in the nip points of the conveyor when he came into close proximity with the rollers of the conveyor while adjusting the conveyor belt of the recycling operation. The employee had gone to the top of the conveyor to check the belt. When he leaned between the motor and the conveyor, his jacket became caught in the conveyor rollers, then he was pulled between the conveyor and the motor, which caused the coat to pull up around the employee's neck, strangling the employee.
