Employee is struck by a falling brick and is killed

At 9:45 a.m. on December 14, 2013, Employee #1, a door installer, was working with a demolition crew to modify a passenger elevator to operate from the tenth floor to the basement. The elevator was currently stopping on the first floor, so the concrete floor of the elevator shaft had to be removed and the basement door had to be rehung. The demoed debris materials were collected into an empty 55 gallon drum. Employee #1 was removing the debris to an outside collection point, which required him to move through a location with two overhead swing stage scaffolds. There were four brick masons on the scaffolds, pointing and repairing the façade. While attempting to move through this area, Employee #1, who was not wearing a hard hat, was struck by a falling brick, fracturing his skull and killing him.
