Forklift operator is crushed and killed when forklift falls

At 2:30 p.m. on December 7, 2013, an employee,who was a temporary worker, was operating a Mitsubishi stand up forklift to load and unload a trailer that was located on a dock. While exiting the trailer to return to the warehouse, the employee inadvertently drove the forklift off the dock. The forklift and the load being carried fell to the ground, crushing the employee beneath. The employee was killed.
