Employee is caught between scissor lift and steel beam and i

At 1:45 p.m. on December 5, 2013, Employee #1 was using a scissor lift to secure a poly tarp around a steel column, prior to spraying fireproofing compound on the steel beam. As The employee was draping the tarp, it became caught on the joystick lever, causing the scissor lift to rise up. Employee #1 was caught between the horizontal steel beam and the top rail of the scissor. The employee was crushed and killed.
