Investigation Summary
At approximately 1:30 p.m. on November 8, 2013, Employee #1 was installing wood siding on a dormer. As he was working, he lost his balance and fell from the roof. Employee #1 struck the ground and sustained multiple injuries including fractures to his C1 and C7 cervical vertebrae and later died.
