Employee is ejected from manlift basket and is killed

At 2:30 p.m. on September 4, 2013, Employee #1 and Employee #2 were in a manlift basket 47 feet above the ground. The manlift basket became caught on two protruding bolts at the end of a concrete cap and ejected the employee and the coworker. Employee #1 was killed from the fall and Employee #2 received several fractured bones, contussions and possibly internal injuries.
