Employee strikes his head operating clamp truck dies

At approximately 2:45 a.m. on August 31, 2013, Employee #1 was loading rolls of paper that averaged 31,500 pounds each. He loaded fifteen rolls of paper into the box of the semi-trailer and was in the process of loading the last roll of paper. The front part of the clamp truck was inside the trailer when the drivers side rear wheel became stuck. Employee #1 leaned over on his left side to see what the rear wheel was caught on. A portion of his head was possibly outside the protective cage of the clamp truck. He pressed on the right side of the directional pedal on the clamp truck which made the clamp truck go forward instead of backwards. His head was caught between the inside of the trailer and the lower fire extinguisher handle located in back of where he was sitting. The wound located on the left side of his head starting bleeding. Since he was alone he drove his clamp truck to another location to find help. He backed up his clamp truck into a roll of paper and fell off the clamp truck onto his left side breaking his ribs. He then rolled over onto his stomach and continued to bleed and was found later unconscious on the floor. Employee #1 later died.
