Employee dies from heart attack; heat stress a contributing

At 11:00 a.m. on July 22, 2011, Employee #1 was working as a loader in Outbound 6, S1D1. Employee #1 began his shift at 5:30 AM that day. The temperature at 5:00 am was 80 degrees Fahrenheit and at 8:00 a.m. it was 82 degrees. At about 8:25 AM Employee #1 walked out of the area into the Revenue Recovery area. Employee #1 was observed by two employees in the Revenue Recovery area making statements that were not logical and saw that he was visibly shaking. Employee #1 rapidly walked away from the Revenue Recovery area. One of the Revenue Recovery employees followed Employee #1, who staggered and collapsed at or near the doorway and was caught by the Revenue Recovery employee. Employee #1 became unresponsive and was convulsing. EMS arrived and administered medical treatment. Employee #1 was transported to La Grange Memorial Hospital and subsequently died. On 09/14/2011, the OSHA Area Office received a revised report stating that the employee died form hypertensive cardiovascular disease and that heat stress was a significant contributing factor.
