Employee Is Struck And Cut By Apparatus On Conveyor And Is K

At 8:30 p.m. on June 21, 2013, an employee was freeing a jammed pallet on a conv eyor in a palletizer machine. The employee removed a loose guard with his hands and reached for the jammed pallet. The employee was struck by the pallet-hoist ing/pallet-lifting apparatus when the photo eye sent a signal to the apparatus t o resume operation. The employee was struck in head and neck and received deep l acerations and was killed.
