Investigation Summary
At 8:30 p.m. on June 21, 2013, an employee was freeing a jammed pallet on a conveyor in a palletizer machine. The employee removed a loose guard with his hands and reached for the jammed pallet. The employee was struck by the pallet-hoisting/pallet-lifting apparatus when the photo eye sent a signal to the apparatus to resume operation. The employee was struck in head and neck and received deep lacerations and was killed.
