At 6:45 p.m. on June 5, 2013, Employee # 1, a truck driver, and Employee #2, coworker, were standing in front of a trailer that was being loaded by a coworker using a forklift. Employee #1 and Employee #2 were pinned against the trailer by the forklift and both were crushed.
