Investigation Summary
At 3:30 p.m. on May 29, 2013, a military technician working at an equipment concentration site shop was performing service / repair operations on / near the engine compartment of a Stewart & Stevenson two and one half ton Light Medium Tactical Vehicle (LMTV) (model M1078, serial number A-T3606BCHE), when unexpected movement of the vehicle cab caused the cab to close suddenly. The sudden closure of the cab pinched the military technicians head between the cab and the front bumper, resulting in severe and fatal head trauma injuries. The military technician was working alone in a shop bay; however, additional employees were working separately on other vehicles in adjacent shop bays. The vehicle cab of the LMTV was and is commonly raised to the upright position while performing service / repair operations on / near the engine compartment. The injured military technician was in the process of performing a repair to the cabs hydraulic system when the accident occurred. No methods of lock out tag out such as but not limited to blocking were utilized prior to the incident.
