Investigation Summary
At 10:55 a.m. on May 21, 2013, an employee was driving a side boom tractor/pipelayer up a steep hill with an approximately 17 degree incline. The employee lost control of the machine as it traveled down the hill striking the trencher machine and landing in the trench crushing the employee in between the trench and the machine killing him.
