Investigation Summary
At 8:15 a.m. on March 18, 2013, Employee #1, employed by an air conditioning/heating/refrigeration contractor, was working from a 10-foot A-frame ladder. He fell from the ladder. Employee #1 was killed in the fall. According to the subsequent investigation report, his injuries included a right leg fracture. There were no witnesses to his fall.
