Employee is crushed and killed by falling steel coil

At approximately 4:37 p.m. on February 20, 2013, Employee #1 was standing on a trailer, 16 foot long by 4 foot wide by 3 foot high, directing a forklift operator in the unloading of a steel coil. The coil weighed approximately 6,633 ponds. The coil was released from the forklift and it rolled toward Employee #1 who jumped off of the trailer. The coil rolled off the trailer and struck Employee #1 fracturing his head and killing him.
