Employee crushed between conveyor and its rollers and is kil

At approximately 8:30 a.m. on February 24, 2013, Employee #1 was using a 1inch by 6 foot metal bar to clean rotating conveyor belt rollers on a moving conveyor belt. He would drag the metal bar across the surface of the rollers rotating towards him. He reached a conveyor belt roller that was too low to the platform to clean from the side of the roller that rotated toward him. He stood on the side of the roller that rotated away from him and when he touched the metal bar to the roller he was drawn through the space between the roller and conveyor belt. Employee #1 was crushed and killed.
