Investigation Summary
EMPLOYEE #1 WAS WORKING ON TOP OF A FOUR STORY MASONRY BUILDING. HE WAS ATTEMPTING TO PLACE A 3/4 INCH THICK, 4 BY 8 FOOT PIECE OF PLYWOOD AS PART OF A NEW ROOF DECK. WHILE ATTEMPTING TO PLACE THE PLYWOOD SHEET, THE SHEET EITHER SLIPPED OR FELL FROM HIS GRASP AND GOT UNDER HIS FEET. EMPLOYEE #1 THEN SLID DOWN THE ROOF WITH THE PLYWOOD SHEET UNDER HIS FEET. HE FELL ABOUT 38 FEET, STRUCK THE FRONT PORCH ROOF, BOUNCED OFF AND FELL ANOTHER 14 FEET TO THE SIDEWALK. HE DIED FROM INJURIES TO THE HEAD, TRUNK AND EXTREMITIES.
