Investigation Summary
EMPLOYEE #1 WAS WORKING ON THE TOP LEVEL OF THE SCAFFOLD ON THE DAY OF THE ACCIDENT (NOVEMBER 4, 1984, 1:40 PM). HE HAD BEEN WORKING AS A FIRE WATCH, MOVING FROM ONE SIDE OF THE RUDDER SCAFFOLDING TO THE OTHER. IMMEDIATELY PRIOR TO THE FALL, EMPLOYEE #1 HAD BEEN STANDING ON THE PORT SIDE OF THE RUDDER SCAFFOLDING, WHICH WAS ABOUT 30 FEET HIGH. THE SCAFFOLDING WAS OLD AND HAD NO MANUFACTURER NAMES OR SERIAL NUMBERS. ONE EMPLOYEE, WHO BELIVES HE SAW THE FATAL EVENT, SAID THAT EMPLOYEE #1 HAD BEEN LEANING AGAINST THE SIDE RAILING WHEN IT GAVE WAY. THE EMPLOYEE FELL TO THE DECKING AND WAS KILLED.
