Investigation Summary
At approximately 7:15 p.m. on January 7, 2013, Employee #1 was preparing a 2010 Frieghtliner tractor for a trip. He was reaching into the cab of the tractor through the open driver's door when the tractor moved forward and the door made contact with a concrete bollard. Employee #1 was caught between the door and the cab of the tractor, in his abdomen area. A coworker found Employee #1 with the keys to the truck in his right hand and facing the windshield. Employee #1 was transported to a local hospital and died.
