Investigation Summary
At 1:45 p.m. on January 7, 2013 an employee was working on a third floor level of a commercial building painting horizontal beams located at the top of glass panels. The employee fell approximately 27 ft through a floor opening where a glass panel was removed for the employees to gain access to the work area. The employee was killed from blunt force trauma to the head and body. The employee was not wearing fall protection equipment and the floor opening was not guarded or covered.
