Employee crushed and killed in trench cave-in

Employee #1 entered an 8 ft deep by 42 in. wide trench to block the end of a pipe. A 2 ft thick by 20 ft long section of the left wall broke away, crushing and killing him. Rock had been blasted from the adjacent 16 ft section of the unshored trench.
