Investigation Summary
At 10:45 a.m. on July 5, 2011 Employee #1 was assigned to hand dig a pipe from the ground, when the employee went to the bathroom outside. After not returning to the work area for over 20 minutes the foreman went to look for Employee #1 and found him outside leaning against the railing and not feeling good. Employee #1 was complaining about chest and back pains. The foreman then called over the second foreman, who in turn called emergency medical services and helped Employee #1 lie down. Employee #1 stopped breathing and responding to anything. The foreman then proceeded to perform cardiopulmonary resuscitation on Employee #1 for about 4 to 5 minutes until the Paramedics arrived and took over and transported him to the hospital, where he was pronounced dead.
