Investigation Summary
At 10:15 a.m. on December 5, 2012, an employee was standing on a step ladder using a drill to dismount a Genset Thermo King diesel fuel powered generator (Model: KIT SGCO 3000, weight: 1890 lbs.) by removing the bolts on the generator from the refrigerated container. Without placing the forklift underneath the Genset, the employee climbed the step ladder to use a drill to remove the bottom bolt and loosen the top bolt on the left hand side of the generator. Then the employee moved the step ladder to the right side and removed the bottom bolt. After loosening the top bolt on the right side, the Genset unexpectedly released from the casting and swung. It made contact with the ladder and knocked it and the employee to the ground. The Genset landed on its side and then fell onto the employee, crushing the head and upper torso.
