Investigation Summary
At 7:00 a.m. on November 13, 2012, an employee was moving pallets of turkeys with an electrical pallet jack to make room for the next load backing up to the dock. The employee was backing up with his hands on the pallet jack when he stepped backwards off the dock and fell approximately 49 inches to the concrete surface. The employee struck his head on the concrete and then was struck by a truck that was backing up, killing him through his injuries.
