CRUSHED BY FALLING PIPE

EMPLOYEE #1 BROUGHT IN A TRUCKLOAD OF PIPE TO BE RECONDITIONED. EMPLOYEE #2 WAS HELPING EMPLOYEE #1 UNLOAD THE PIPE. EMPLOYEE #1 WAS EMPLOYED BY THE CLIENT COMPANY. EMPLOYEE #2 IS EMPLOYED BY THE RECONDITIONING COMPANY. THE TWO EMPLOYEES WERE ON THE DRIVER'S SIDE OF THE TRUCK FLOAT. THEY HAD RELEASED 3 OF THE 4 CHAIN BOOMERS HOLDING THE LOAD OF PIPE WHEN THE LOAD SHIFTED. EIGHT LENGTHS OF PIPE, EACH 16 INCHES IN DIAMETER AND 20 FEET LONG, FELL OFF THE TRUCK. EMPLOYEE #2 WAS STRUCK AND KNOCKED AWAY FROM THE FLOAT. SOME OF THE PIPE FELL ON TOP OF EMPLOYEE #1. EMPLOYEE #2 WAS PRONOUNCED DEAD AT THE SCENE. EMPLOYEE #1 WAS HOSPITALIZED, BUT DIED FOUR WEEKS LATER WITHOUT REGAINING CONCIOUSNESS. THE TRUCK TRACTOR WAS A GMC CAB-OVER, PULLING THE TRAILER FLOAT LOADED WITH PIPE.
