Investigation Summary
ON 1/11/85 AT APPROXIMATELY 2:15 PM EMPLOYEE #1 WAS BUILDING A PARTITION FOR AN OFFICE AND WORKSHOP. HE WAS USING A DEFECTIVE 6 FOOT A FRAME ALUMINUM LADDER WHICH WAS INITIALLY HELD BY ANTHER EMPLOYEE. THE HORIZONTAL SUPPORTS OF THE LADDER WERE LOOSE, CAUSING THE LADDER TO BE SHAKEY. WHEN DECENDING THE LADDER IT EITHER TWISTED OR BROKE, CAUSING THE DECEASED TO FALL. HE FELL FROM A DISTANCE OF APPOXIMATELY 4-5 FEET, HIT HIS HEAD ON THE CONCRETE FLOOR, AND WAS KILLED.
