Investigation Summary
At approximately 1:15 p.m. on September 9, 2012, Employee #1 was in a trench. He signaled the operator of a Komatsu PC400 LC track hoe into position. The company was installing a 36-in. pipe in a trench approximately 12 ft deep. The section of pipe being put on was 20 ft long and had a collar attached for a 22-degree turn to go under an existing 24-in. and 30-in. pipe already in the trench. The pipe was being suspended from a sling on a track hoe approximately 6 in. off the bottom of the trench. Another track hoe being driving by the operator was using its excavator bucket to push the pipe connection together. After Employee #1 positioned the excavator, he moved forward away from the bucket and then the bucket slipped. Employee #1 was struck in the chest by the excavator bucket and pinned against an existing 24-inch water line. He sustained crushing injuries and died.
