Event: 11/18/1985
CRANE OPERATOR SUFFERED HEART ATTACK

ON NOVEMBER 18, 1985, AT APPROXIMATELY 6:10 AM, EMPLOYEE #1 WAS SEATED AT THE CONTROLS OF A LIEBHERR 250C C-TYPE ELECTRIC TOWER SLEWING CRANE, LOCATED AT PIER 5, ROUTE 2, SOUTH WEIRTON, WEST VIRGINIA. EMPLOYEE #1 HAD LOWERED A WORK PLATFORM TO THE DECK OF THE PIER. HE WAS GOING TO HOIST MEN AND EQUIPMENT TO THE PIER AREA UNDER CONSTRUCTION. WHILE THE WORK PLATFORM WAS BEING LOADED WITH EQUIPMENT, EMPLOYEE #1 APPARENTLY SUFFERED A CARDIAC ARREST. HE FELL FORWARD AND TO THE LEFT. HIS RIGHT SHOULDER STRUCK THE LEFT (SLEW) CONTROL, FORCING THE CRANE TO JERK THE WORK PLATFORM INTO THE AIR AND 65 FEET TO THE LEFT, WHERE IT SLAMMED INTO THE CONCRETE PIER. THIS SLEWING ACTION APPARENTLY FORCED THE LEFT FOOT OF EMPLOYEE #1 AWAY FROM THE FLOOR BAR CONTROL. THE RELEASE OF THE BAR ACTIVATED THE DEAD MAN CONTROL AND BROUGHT THE CRANE TO A HALT. THE PROJECT MANAGER THEN TRIED TO COMMUNICATE BY RADIO WITH EMPLOYEE #1. WHEN HE GOT NO RESPONSE, HE DISPATCHED A SECOND CRANE OPERATOR TO THE CRANE'S OPERATOR STATION. THE SECOND CRANE OPERATOR FOUND EMPLOYEE #1 SLUMPED FORWARD OVER THE CONTROLS. EFFORTS TO REVIVE EMPLOYEE #1 WERE NOT SUCCESSFUL.
