Investigation Summary
At 2:30 p.m. on September 6, 2012, Employee #1 was working at a farm that houses several bulls for the purpose of collecting semen for artificial insemination of dairy cattle. Employee #1 and his coworker were assigned cleaning bull waste with a skid steer. Before beginning work, Employee #1 and his coworker secured the bull in one section of the pen and drove the skid steer into each stall for cleaning. After each stall was cleaned, they were instructed to open the gates into their original positions. This required opening the gates one-by-one and closing them to form a large rectangular pen for the bull to occupy. As Employee #1 and his coworker were positioned on the outside of the hinged gate of one of the stall and were walking backwards to secure the gate in its original position, the bull rushed the gate. Employee #1 became pinned between the end of the gate and the fixed four inch steel corner post. Employee #1 was caught between the gate and the corner post, resulting in crushing injuries to his chest and skull. Employee #1 was taken to a local hospital, where he died approximately 30 minutes later.
